Public Health Nurse Home Visiting
Through the Baby and Me Nurse Home Visiting Program, the PHN performs an assessment of a pregnant woman’s understanding or compliance with medical recommendations, support system for mother and baby, home situation, care and feeding of the baby including breastfeeding techniques, community information and referrals (food housing, WIC, transportation) and health care follow up. The program also provides education on pre/post natal concerns and infant care. The nurse is able to provide a developmental screening and weigh and measure the baby at each visit. Referrals from medical providers or community organizations can be made for a variety of reasons including medical concerns (diabetes, pregnancy, history of premature delivery), late entry into prenatal care, teen pregnancy, history of substance abuse or mental health concerns. Women may also self-refer.
Prenatal Care Guidance
Women are linked to resources such as OB providers, health coverage, prenatal education, nutrition services and to resources that can provide assistance for housing, utilities and transportation. The PHN also assists with Medi-Cal application.
SIDS
Support and follow up are available to families and caregivers who experience a Sudden Infant Death. This includes home visits and referrals for assistance for the affected family and outreach and education to those in the community who have a connection to a SIDS death such as child care providers.
