For Medical Providers

Title 17 of the California Code of Regulations requires all health care providers to report to the Local Health Jurisdiction (LHJ) all cases of the communicable diseases listed on the Confidential Morbidity Report (CMR) form.

Diseases of a critical nature require urgent reporting by telephone. All other communicable diseases may be reported by fax using the CMR. The CMR may be completed online and printed. It cannot be saved once completed; nor submitted electronically.

There are three separate CMR forms. One form is specifically for tuberculosis (TB); one form is specific for lapse of consciousness reporting; and the other form is for all other reportable conditions.

Of note, Calaveras County requires reporting of all positive (+) TB skin tests (TBST or PPD) to Calaveras Public Health via the TB CMR.
