Child Health & Disability Program (CHDP) Information

Eligibility for CHDP includes:

• Family income at or below 250% of the Federal Poverty Level or
• Children covered by Emergency Only Medi-Cal

Monday through Friday from 8AM – 5PM

There is no cost for children's preventive health screenings for CHDP eligible families.

Children and youth to age 21 who are low income or have Medi-Cal can receive CHDP services through local enrolled medical providers.
